We are delighted to collaborate with the Asia Diabetes Foundation to support the Jockey Club Precision Prevention Programme for Young Onset Diabetes (PPP-YOD).

Programme Overview

The Jockey Club Precision Prevention Programme for Young Onset Diabetes is Hong Kong's first initiative to combine biological genetic markers with traditional risk factors to identify individuals aged 18 to 44 who are at high risk of early onset diabetes. Through advanced genetic testing and comprehensive health screenings, the programme aims to provide personalised interventions to prevent or delay the onset of diabetes.

Eligible individuals must meet the following criteria:

  • Aged between 18 and 44
  • Currently do not have diabetes
  • Have at least one diabetes risk factor*

Participants will undergo risk screenings conducted by nurses, including:

  • Diabetes risk assessment
  • Capillary blood glucose test
  • Saliva collection for DNA test
  • Health knowledge and lifestyle assessment

Personalised Risk-Based Interventions

Participants identified as high-risk for diabetes will be scheduled for blood tests and urine tests at designated locations. Based on the risk assessment results, participants will be categorised into different risk groups and provided with tailored interventions over a two-year period. A doctor-nurse team will work closely together to offer comprehensive diabetes support. The two-year programme includes the following:

1. High-Risk Group

  • 3 Sessions with Family Doctor: To explain the risk report and provide personalised advice.
  • 4 Follow-Up Sessions with Family Doctor: Including subsidised medication support if needed.
  • 4 nurse consultation: For ongoing monitoring and support.
  • 3 Comprehensive Health Checks: Including BMI, blood pressure, blood tests, and urine tests.
  • 3 Diabetes Risk Assessments
  • 1 Body Weight Scale Subsidy

2. Impaired Glucose Tolerance Group

  • 3 Sessions with a Family Doctor: For detailed report explanations and guidance.
  • 8 Follow-Up Sessions with a Family Doctor: Including subsidised medication support if needed.
  • 4 nurse consultation
  • 3 Comprehensive Health Checks: Including BMI, blood pressure, blood tests, and urine tests.
  • 3 Diabetes Risk Assessments: To monitor changes in risk over time.
  • 1 Body Weight Scale Subsidy: To promote healthy weight management.
  • 1 Continuous Glucose Monitoring (CGM) Device Subsidy: For real-time glucose tracking.

3. Diabetes Group

  • 3 Sessions with a Family Doctor: To discuss the risk report and develop a management plan.
  • 12 Follow-Up Sessions with a Family Doctor: Including subsidised medication support if needed.
  • 4 Nurse Follow-Up Sessions: For ongoing care and education.
  • 3 JADE@ Asia Diabetes Evaluation Programme Reports: To assess diabetes control and complications.
  • 3 Diabetes Complication Risk Assessments: Including eye and foot examinations, BMI, blood pressure, blood tests, and urine tests.
  • 1 Body Weight Scale Subsidy
  • 3 Continuous Glucose Monitoring (CGM) Device Subsidies: For advanced glucose monitoring.
